Online

You may register with MasterCard, Visa or American Express using 5 Level Training’s secure online ordering system. Only credit cards are accepted for online orders. To use a Purchase Order, see below.

By Phone

If you prefer to register by phone, you may call 5 Level Training at 1.636.629.1928. We accept MasterCard, Visa and American Express. Purchase orders, checks and cash are not accepted for phone registrations.

By Mail

We accept credit cards, checks, and purchase orders through the mail. Simply fill out our convenient printable registration form (.pdf) and mail it to:

By Fax

We accept credit cards and purchase orders via fax. Please fax us at 1-607-428-6350 using the printable registration form (.pdf)

Purchase Orders

Purchase order transactions cannot be accepted online or by phone. To use a purchase order, mail or fax the purchase order and printable registration form to 5 Level Training. Purchase orders must be on your company's letterhead. First-time participant registrations must be prepaid.

Internet Security

Registrations and payments are processed with Secure Sockets Layer (SSL) software to secure online transactions by encrypting all of your personal information, including your credit card number, name, and address, so that it cannot be read as the information travels over the Internet.

Credit Card Security

5 Level Training does not store credit card information.

Cancellation Policy

If you need to cancel for any reason, please notify us in writing at 5 Level Training, 1324 Clarkson Clayton Center, Suite 310, St. Louis, MO 63011 or fax to 607-428-6350 or email Registration@5LevelTraining.com.

You will receive a full refund if your cancellation is made more than 30 days prior to the program. If you cancel within 1-30 days of the program date, you will receive either a full credit toward another training program or a refund less a 25% processing fee. Of course you may transfer your registration to another person prior to the start of the program.

Any cancellations made after the program begins are non-refundable.

5 Level Training reserves the right to change speakers or reschedule/cancel sessions when necessary. We are not responsible for airfare penalties that may be incurred due to cancellation of any program.
